There are several ways to apply for admission within the facility, depending on your needs.
Subacute Admissions
To help you prepare for hospital discharge, our case manager will visit with you there for an initial evaluation, assessing your potential for progress as we develop your care plan (called a clinical path). Our interdisciplinary team works closely with your family doctor to assess your medical requirements, as well as your capacity to handle the activities of daily living (or ADLs). This evaluation includes an estimated discharge based on your individual needs and discharge criteria.
Long-Term Care Admissions
Our long-term care admissions are afforded a personal tour of the facility and are encouraged to ask any questions they may have. At that time, rates and other financial matters can be clarified. Patients' primary care physician will need to forward their medical history so that our staff can be provided with accurate information to assess and create customized treatment plans.
